About Potters House Christian Academy - upper campus

Along with your pre-registration and registration packets, you will need an up-to-date Florida Immunization Certificate (Form 680), a recent Student Health Examination Form (Form DH 3040), dated within the current school year, a copy of his/her birth certificate, and a copy of the child's social security card. Additionally, students enrolling in grades 7 - 12 will need character references and a copy of the transcript with any high school credits.

At the upper school lever, once the information requested above is reviewed, you will be contacted to set up an appointment for an interview and testing if deemed necessary. The exam consists of both academic assessment and scholastic ability. At the time of testing a $30 testing fee made payable to PHCA is required.

Automatic Withdrawal (Non-scholarship parents)
Tuition payments must be made through PHCA Direct Draft. The Family Registration Fee is $25. Parents may select the 5th or the 20th as their draft date.

Scholarship
Our school currently accepts the Step Up for Students Scholarship. Once parents have received their award letter, they may stop by the school office to sign their commitment form. To apply for the Step Up for Student Scholarship, log onto www.scholarshipfunding.org and complete the application.
The average award amount for 2011 - 2012 school year is $4011. Partial scholarships are also available. All academy fees not covered by scholarship must be paid at time of registration.

Jacksonville Children Commission
Jacksonville Children's Commission offers compensation for qualifying parents to assist in covering a portion of extended day fees and/or tuition for students up to age 5. Application must be made directly to JCC. Qualifying parents must present their award letter to the school to determine their out of pocket cost (parent fee). This parent fee is subject to the same guidelines as regular tuition payments. It is due on the first of each month or a $30 late fee will be assessed after the fifth of each month.

Because our tuition is among the lowest in the city, a multi-family discount is not available at this time.
